Title: Empowering Systemic Health by Mouth: See & Treat Impaired Mouth Syndrome

Title: The Oral and Systemic Disease Linked to Porphyromonas gingivalis

Abstract: Porphyromonas gingivalis is a high-risk pathogen highly associated with chronic periodontitis. The biology of this organism is uniquely suited to evoke inflammation locally and systemically, while also possessing the ability to invade tissue. Infection by P gingivalis of the periodontium can, in some persons lead to the systemic colonization in remote organs of the body including the brain, where over long periods of time assert the effect locally to degrade neurons and supportive tissue and leading to clinical signs identical to Alzheimer’s disease. Research into the pathophysiology of systemic infection involving P. gingivalis have subsequently led to the identification of novel compounds now being evaluated as potential drug therapies for AD. Along with this work is the need for new diagnostic tests that can precisely measure the quantity P gingivalis in samples of oral rinse to be used in clinical trials to slow or reverse the progression of cognitive decline in dementia patients. A summary of those trials methods to test and the findings of the drug trials will be presented.

Objectives:

  1. This talk seeks to present an overview of the biology of the high-risk pathogen Porphyromonas gingivalis
  2. This talk will detail the approach to the molecular detection of P gingivalis in the clinical laboratory and the cumulative findings based on applications for patient care.
  3. This talk will present the findings of a provocative clinical trial testing a new drug for the treatment of Alzheimer’s disease associated with P. gingivalis.
  4. This talk will highlight innovations in technology for chair side diagnostics in dentistry exemplified by a new test for P. gingivalis.

Ronald McGlennen, MD is the founder of Access Genetics and OralDNA Labs, where he serves as its president and medical director. Dr. McGlennen is board certified in anatomic and clinical pathology as well in clinical molecular genetics. Previously, he served as an Associate Professor of Pathology at the University of Minnesota Medical School where he founded the molecular diagnostic laboratory until 2003. As founder of Access Genetics his mission was to accelerate the availability, timeliness and quality of molecular genetic testing for patient care through technical innovations including the pioneering use of telemedicine.    He has published more than 89 scientific articles and book chapters and has served as an editor of five professional journals. He holds 9 issued patents. He also serves on the board of directors of several early-stage companies in the diagnostic space. His focus in research has been on reducing the complexity of gene-based testing, including DNA chip technology and simple analytic instrumentation to better serve the community laboratory.

Consequently, he is internationally recognized as an expert in molecular biology genetics and the technologies for diagnostic medicine. In recent years he and his company have forged the path to bring laboratory diagnostics to the oral health professions with innovations in salivary testing. Barbara K. Tritz RDH, BS, MSB is a registered dental hygienist with over 40 years of clinical experience. She practices in the office of Green City Dental in Edmonds, WA. She is also owner of Washington Oral Wellness in Kirkland, WA and is a practicing orofacial myofunctional therapist. Barbara received her AS degree in dental hygiene from Onondaga Community College in Syracuse, NY, a Bachelor of Science in dental auxiliary teacher education from the University of North Carolina, Chapel Hill, and a Master of Science in Business from Husson University in Bangor Maine. She continued her training in myofunctional therapy from the Academy of Orofacial Myofunctional Therapy in Los Angeles. Title: The Best Available Scientific Evidence Supports Fluoride Use for Caries Prevention and Control

Presented by: Dr. Scott L. Tomar

Community water fluoridation is a safe and effective public health measure for dental caries prevention. Following the widespread demonstration of its prevention effectiveness, fluoride was incorporated into toothpaste, mouthrinses, and other topical agents. Contemporary high-level evidence strongly supports the use of topical fluorides for dental caries prevention and control, in conjunction with measures to control other established risk factors for this multifactorial disease.

Objectives:
At the conclusion of this course, the participant will be able to:

  1. Discuss the conclusions of major systematic reviews on the effectiveness and safety of community water fluoridation for the prevention of dental caries.
  2. Describe evidence-based clinical recommendations from major dental organizations for the use of topical fluorides in caries prevention and control.
  3. Understand the role of fluorides within a more comprehensive approach to dental caries prevention and control.

Bio:
Dr. Scott L. Tomar is public health dentist, epidemiologist, professor, and Associate Dean for Prevention and Public Health Sciences at the University of Illinois Chicago College of Dentistry, where he also serves as Interim Associate Dean for Research and Interim Head of the Department of Oral Biology. He is currently President of the American Board of Dental Public Health and is a member of the American Dental Association’s National Fluoridation Advisory Committee. Dr. Tomar has published about 180 journal articles and book chapters, and contributed to seven reports of the US Surgeon General. He is a former editor of Journal of Public Health Dentistry and Journal of Evidence-Based Dental Practice and has received numerous awards, including IADR’s H. Trendley Dean Distinguished Scientist Award. Dr. Tomar served as President of the Oral Health Section of the American Public Health Association (APHA), the American Association of Public Health Dentistry (AAPHD), and the Behavioral, Epidemiologic and Health Services Research Group of the International Association for Dental Research (IADR). Dr. Tomar received his B.A. and D.M.D. degrees from Temple University, M.P.H. degree from Columbia University, and certificate in Dental Public Health and Dr.P.H. degree in Oral Epidemiology from The University of Michigan. Dr. Tomar served as an Epidemic Intelligence Service Officer and Epidemiologist with the Centers for Disease Prevention and Control and is a Diplomate of the American Board of Dental Public Health. Dr. Campbell is a Double Board-Certified Physician with clinical interests in family medicine and holistic care.​

Originally a native of metro Chicagoland, she graduated from the University of Illinois in 1983, and obtained a Master of Science degree in 1984. She obtained her DO degree from the Kirksville College of Osteopathic Medicine in 1989, emphasizing care for the body, mind and spirit. Retreating to a southern climate, she then moved to Georgia for her Family Medicine training. In 1992, she received the outstanding resident of the year award from the Family Medicine department at the Medical College of Georgia in Augusta. For the first thirteen years of her career, Dr. Campbell served the uninsured of our community practicing at a community health center. In 2005 she started her own private practice. In her free time, Dr. Campbell and her husband vacation with their three wonderful daughters and enjoy theater, yard work and gardening together.

Dr. Campbell is also the trusted physician to many physicians, surgeons, dentists, chiropractors, veterinarians, and their families.

Nicki Mackey is a marketing professional who is passionate about oral systemic health. She gained her Fellowship with the American Academy for Oral Systemic Health (AAOSH) in 2022, and is one of only two non-clinical professionals to do so. Nicki is the founder and CEO of Redwood Designs, a design and marketing company specifically tailored to the prevention-focused medical and dental community. Redwood Designs brings this systemic health expertise to its clients, creating compelling written and visual language that is accessible to both the healthcare professional and layperson.
Title: You are more than what you eat: classifying obesity using genomics and metabolomics
 
Abstract: Obesity is a complex disorder affecting up to 40% of the American population. More than a condition of control of food and caloric intake, recent studies support that varied physiologic pathways may be dysregulated in persons who are overweight. This means that there is no single mechanism by which obesity can be explained. Understanding the genetic propensities, and the subsequent altering of physiologic pathways has led to our ability to classify the causes of obesity. Testing persons by means of genetic analysis or by use of methods for metabolomics can offer a precision diagnostic to help caregivers then apply the more correct therapies. This lecture will introduce the methods of study, the results of recent studies and the forecast of new therapies in the management of obesity, arguably one of the greatest health concerns of the Western populations.
 
Learning Objectives: 
1.This talk will outline the current state of knowledge on the nature or cause of obesity, involving varied physiologic pathways different in differing people.This knowledge explains, in part why many treatment approaches fail and why laboratory testing will offer more correct diagnoses.
 
2.This talk seeks to present the finding from a genetic wide association study (GWAS) revealing how gene-based testing can more correctly classify the cause of obesity for individual patients, thereby directing therapy to intervene in the affect physiologic pathway.3.This talk will also introduce the precepts of metabolomics, a branch of diagnostic sciences that employs the measurement of the products of cellular metabolism to characterize health and conditions of disease. Using mass spectroscopy to quantify amino acid profiles in blood can genetic testing and further broaden the classification of the causes of obesity.4.This talk will present the utility of these new diagnostic approaches to then personalize therapy for obesity, with a focus on which patients are more likely to succeed with drugs such as the glucagon-like peptide (GLP-1) agonists.

Andres J. Acosta, M.D., Ph.D. is a consultant in theDivision of Gastroenterology and Hepatology, Department of Internal Medicine at the Mayo Clinic in Rochester, Minnesota where he also serves as an assistant professor.  His research focus is in the area of gastrointestinal physiology to understand the complexity of food intake regulation and obesity. Obesity is a primary cause of type 2 diabetes, fatty liver disease, cardiovascular disease and cancer. Dr. Acosta's laboratory utilizes a combination of genetics, physiology, pharmacology, proteomics, metabolomics, and gastrointestinal and brain imaging to understand food intake regulation and to modulate them for the treatment of obesity. In collaboration with his mentor, Michael Camilleri, M.D., Dr. Acosta is using cutting-edge techniques to understand the pharmacotherapy and pharmacogenomics of obesity. Dr. Acosta's main focus is on the mechanisms and regulation of appetite, satiation and satiety, including the role of gastric function, gastrointestinal hormones, taste preferences, bile acids, andenterohepatic regulation and gut energy utilization. Additionally, Dr. Acosta studies the effect of genetics, pharmacology and endoscopic devices on food intake regulation.

Dr. Shalom Samuel grew up in Chennai, India and moved to the United States with her family at the age of 12. After graduating from West Bend high school in Wisconsin, she entered the Pre-Med program at UW-Parkside and had the goal of going to medical school to become a Dermatologist. During her junior year of college, she was awarded an exceptional scholarship to go study abroad in Australia at Macquarie University for a semester. Upon successful completion, she continued her studies & graduated in 2008 with a Bachelor of Science Degree (B.S). During her time at Parkside, Dr. Shalom was actively engaged in church activities, mission trips to Mexico and India, had leadership roles in various clubs and organizations including the Pre-Med and Biology clubs.
